The Healthier Community Partnership Initiative (HCP) is Fraser Health’s commitment to working with municipalities and community organizations to build healthy communities.

Aligned with the high priority initiatives in Fraser Health’s 2013-2015 Strategic Imperatives, the HCP initiative aims to increase municipal collaboration and enhance internal and external partnerships to create healthy environments where citizens live, work, learn, and play.

Through collaboration among various community organizations HCP facilitates cross-sector action and leads to a greater impact on the health of all residents and ultimately, aims to reduce the risk of chronic disease. Fraser Health provides support to local HCP groups through a dedicated team of health professionals in each community. This team includes an Executive Director, a Medical Health Officer and a Community Health Specialist. Through shared leadership between local government and Fraser Health, the HCP group works to build community capacity and support specific action planning to promote health. This team also serves as a link between Fraser Health program areas and the municipality as required by specific community activities.

The HCP framework identifies selected priorities for action to create healthier communities based on recommendations from the Fraser Health Medical Health Officers’ Annual Population Health Report 2010 and on best and promising practice from other jurisdictions. The strategies provide an upstream approach by focusing on policy, public awareness and initiatives that create supportive environments rather than centering on individual behaviour change.

Areas for collective action include:

  • Healthy Eating
  • Physical Activity
  • Tobacco Reduction
  • Healthy Built Environment
  • Vulnerable Citizens and Neighbourhoods

While these are suggested priorities, each community creates their own “Community Action Plan” based on the knowledge of “what works”, the specific needs of their community and the strengths of the partners at the HCP table.  

Fraser Health’s HCP team assists them by bringing best practice expertise and knowledge in the development, implementation and evaluation of their Community Action Plans.

To complement work being done by the HCP team, a Healthy Built Environment contingent comprised of trained Health Protection staff has been formed to support Fraser Health communities.
